Components of a Calibration Certificate

EXAMPLE CERTIFICATE
Title, Where, What

The first block of information should contain the following:

• Title and Unique ID for the document/page
• Name and address of the calibration lab doing the calibration
• Title of the document (i.e. Certificate of Calibration)
• Type of instrument being calibrated
• Identity of the instrument
• Name of the customer contact
• Name and address of customer's company
