Components of a Calibration Certificate

ISO-17025 Requirements

The requirements of ISO 17025 must include answers to the questions, what was calibrated, when and where this calibration was performed, who performed the calibration, and how was it performed. ISO 17025 Sections 5.10.2 and 5.10.4 cover the specific requirements for test reports and certificates of calibration.

WHAT?
A title for the certificate
A unique identifier for the document including page numbers
A unique identifier on the equipment that connects it to the calibration certificate
Name and address of customer
Description of item being calibrated

WHEN?
Simply the date the unit was calibrated

WHERE?
The name and address of the calibration lab providing the service and location of calibration if different from listed lab

WHO?
ID or signature of person doing the calibration

HOW?
The method or procedure used for calibration must be referenced. Other components such as accuracies, units of measurement, standards used, evidence of traceability, environmental conditions, and uncertainties are discussed in this section.
